Heart Attack Risk Calculator

I developed this simple Heart Attack Calculator to appreciate approximately one's risk of having a Heart Attack. It calculates and appreciates data about your actual health situation and your hereditary factors and due to some algorithms; it appreciates in percents what's your risk of having a Heart Attack at this moment.
I think this tool will become useful and you will appreciate my work. This calculator is illustrative; it doesn't substitutes your doctor's diagnosis and decisions.
